
Liability – Auto - Property LOSS NOTICE INSTRUCTIONS
A Loss Notice must be submitted for all Liability-Auto-Property claims. Our receipt of your Loss Notice initiates our claims-handling process. Do not send attorney’s letters or police reports without an accompanying Loss Notice.
- For routine claims, complete, and submit the Loss Notice within 24 hours of the accident or when you first learn of any incident, occurrence, or potential claim.
Do not wait to receive written notification of the accident, police or hospital reports, or written notification of a claim before filing a Loss Notice with CIRMA.
For serious claims, such as a fatality, serious injury, or first-party property loss, call CIRMA immediately to report the claim:
- During business hours, call (800)-526-1647;
- After business hours, call (203) 710-4479.
Then complete and submit a Loss Notice as soon as possible.

- When you complete the Loss Notice, attach all relevant documentation. This may include police reports, photographs, letters, internal memos, work logs, etc. List these attachments in the REMARKS section of the Notice.
- List all known witnesses in the WITNESSES section of the Notice.
- The Loss Notice must be completed by an authorized CIRMA member, signed and dated, then mailed, faxed or emailed to:

- Once a claim file is set up, CIRMA will send an acknowledgement letter to the member indicating the claim number and the claims representative assigned to the file.
Remember - Never Accept Responsibility or Offer Payment for a Claim. Obtain Information and Inform the Claimant that It Will Be Reported to The Insurance Carrier and That the Carrier Will Be in Contact.
