SUMMARY

This workshop will teach you how to conduct a comprehensive accident investigation. It discusses the reasons why accidents should be investigated, the types of accidents that must be investigated, and when an accident investigation should be conducted. The workshop also discusses who should perform the investigation.

LEARNING OBJECTIVES

Supervisors play a key role in accident investigations for employees who are injured on the job. The purpose of an accident investigation is to prevent the recurrence of the accident. The focus should be on the accident, not the injury. Unfortunately, too often, the investigation process is limited to the necessary insurance paperwork.

Accident investigations are critical in identifying, controlling and eliminating workplace health and safety risks. For an accident investigation to be effective it needs to be structured and systematic. It also needs to be performed by trained employees who can maintain an open mind about the causes of an accident throughout the investigation. These are fundamental requirements of an accident investigation.

The primary learning objectives of this workshop are to:

  • Understand the value of and need for accident investigation.
  • Obtain the practical supervisory skills to conduct quality accident investigations.
  • Understand the critical role of the supervisor in the accident investigation and reporting process.
  • Learn how to determine the root cause(s) of accidents and develop accident prevention action.
  • Understand the CIRMA claims management function and how investigations are critical to their success.

Mark Wantroba
Senior Risk Consultant, CIRMA

Mark serves as a Senior Risk Control Consultant for CIRMA’s Risk Management Services. He has over twenty years experience working in the commercial insurance industry providing risk management and safety and health services. Mark holds a B.S. Degree in Biology with a minor in Chemistry from Southern Connecticut State University. He also completed his M.S. in Exercise Physiology (Biomechanics) from the same institution. Mark is a Certified Safety Professional and holds the Associate in Risk Management and Associate in Loss Control Management designations. He has been a member of the American Society of Safety Engineers since 1989.

From I-91 Southbound:

Take Exit 12. Turn left (South) onto Washington Avenue. Go approximately 1 mile to traffic light at Broadway. Turn right on Broadway. At next traffic light turn left onto Elm Street. Library parking lot is the first left.

From I-91 Northbound:

Take Exit 11. Bear right off the exit ramp. At first traffic light turn right onto Washington Avenue. At next traffic light turn right onto Broadway. At next traffic light turn left onto Elm Street. Library parking lot is the first left.

From Rt. 15 (Wilbur Cross Parkway) Southbound:

Take Exit 63. Turn left onto Hartford Turnpike. At traffic light, turn left onto Bishop Street (Rt. 22). At 2nd traffic light, turn right onto State Street. Go one block to stop signs. Turn left onto Broadway. At the first traffic light, turn right onto Elm Street. Library parking lot is the first left.

From Rt. 15 (Wilbur Cross Parkway) Northbound:

Take Exit 63. Turn right at end of exit onto Bishop Street (Rt. 22). At next intersection, turn right onto State Street. At next intersection (stop signs) turn left onto Broadway. At the first traffic light, turn right onto Elm Street. Library parking lot is the first left.

From Rt. 22 (Northford):

Turn left onto Washington Avenue. At next traffic light turn right onto Broadway. At the first traffic light, turn right onto Elm Street. Library parking lot is the first left.

From Rt. 40 (Hamden):

Take the Bailey Road exit. Turn left at traffic light onto Elm Street. The Library is near the end of Elm Street on the right. (Across from the cemetery) Parking lot is the last driveway on the right
