SUMMARY
This workshop will teach you how to conduct a comprehensive
accident investigation. It discusses the reasons why accidents
should be investigated, the types of accidents that must be
investigated, and when an accident investigation should be
conducted. The workshop also discusses who should perform
the investigation.

LEARNING OBJECTIVES
Supervisors play a key role in accident investigations for
employees who are injured on the job. The purpose of an accident
investigation is to prevent the recurrence of the accident. The
focus should be on the accident, not the injury. Unfortunately,
too often, the investigation process is limited to the necessary
insurance paperwork.
Accident investigations are critical in identifying, controlling
and eliminating workplace health and safety risks. For an
accident investigation to be effective it needs to be structured
and systematic. It also needs to be performed by trained
employees who can maintain an open mind about the causes of
an accident throughout the investigation. These are fundamental
requirements of an accident investigation.
The primary learning objectives of this workshop are to:
- Understand the value of and need for accident investigation.
- Obtain the practical supervisory skills to conduct quality
accident investigations.
- Understand the critical role of the supervisor in the accident
investigation and reporting process.
- Learn how to determine the root cause(s) of accidents and
develop accident prevention action.
- Understand the CIRMA claims management function and
how investigations are critical to their success.

Risk Management Field, Joey has provided
Risk Management consulting to various
public agencies that range from Public Housing Authorities
to Section 8 and low income housing across the country focusing
on General Liability, Fleet and Property. Joey also has experience
in Security and Emergency Medical Response as he spent 5
years as a volunteer within his home town's Fire Department.
Joey has a Bachelor's of Arts Degree in Sociology with a
Concentration in Criminal Justice from Central Connecticut
State University and has been certified as a Public Housing
Manager, a Licensed Producer, and a Licensed Casualty and
Property Adjuster.

DIRECTIONS
From the HARTFORD Area:
Take 91 South to Exit 22s, Route 9. Proceed down Route 9 and take
Exit 15 - Washington Street (Route 66/DeKoven Drive Exit). At light,
take right on to Washington Street then quick left on to DeKoven Drive.
Take next right onto Court Street, then first right on to Melilli Plaza.
Municipal Building is first building on right and parking is located
in rear of building.
From the NEW HAVEN Area:
Take 91 North to Route 66 East (Washington Street - Exit 18), following
signs to Middletown. Proceed down Route 66 until you reach Main
Street. Go through light and take first right on to Melilli Plaza.
Municipal Building is located at end of road on left and the parking
lot is in the rear of the building.
From the PORTLAND Area:
Take Route 66 East and proceed over the Portland Bridge into
Middletown - this turns into Main Street. At intersection of Main and
Washington Street, take left on to Washington Street then first right
on to Melilli Plaza- Municipal Building is located at the end of the
road on the left and the parking lot is in the rear of the building.
From the SHORELINE Area:
Take Route 9 North and take Exit 15 for 66 West, "Middletown/
Meriden." At light, take left on to Washington Street then quick left
on to DeKoven Drive. Take next right on to Court Street, then first
right on to Melilli Plaza. Municipal Building is first building on right
and the parking lot is located in the rear of building.
