SUMMARY
This workshop will teach you how to conduct a comprehensive
accident investigation. It discusses the reasons why accidents
should be investigated, the types of accidents that must be
investigated, and when an accident investigation should be
conducted. The workshop also discusses who should perform
the investigation.

LEARNING OBJECTIVES
Supervisors play a key role in accident investigations for
employees who are injured on the job. The purpose of an accident
investigation is to prevent the recurrence of the accident. The
focus should be on the accident, not the injury. Unfortunately,
too often, the investigation process is limited to the necessary
insurance paperwork.
Accident investigations are critical in identifying, controlling
and eliminating workplace health and safety risks. For an
accident investigation to be effective it needs to be structured
and systematic. It also needs to be performed by trained
employees who can maintain an open mind about the causes of
an accident throughout the investigation. These are fundamental
requirements of an accident investigation.
The primary learning objectives of this workshop are to:
- Understand the value of and need for accident investigation.
- Obtain the practical supervisory skills to conduct quality
accident investigations.
- Understand the critical role of the supervisor in the accident
investigation and reporting process.
- Learn how to determine the root cause(s) of accidents and
develop accident prevention action.
- Understand the CIRMA claims management function and
how investigations are critical to their success.

Risk Management consulting to various
public agencies that range from Public Housing Authorities
to Section 8 and low income housing across the country focusing
on General Liability, Fleet and Property. Joey also has experience
in Security and Emergency Medical Response as he spent 5
years as a volunteer within his home town's Fire Department.
Joey has a Bachelor's of Arts Degree in Sociology with a
Concentration in Criminal Justice from Central Connecticut
State University and has been certified as a Public Housing
Manager, a Licensed Producer, and a Licensed Casualty and
Property Adjuster.

DIRECTIONS
From Points WEST, Waterbury and Danbury:
Take I-84 east through Hartford to Route 384 (Exit 59). Pick up
Route 6 in Bolton. At the intersection of Routes 6 and 66 in
Columbia, continue (left) on Route 6 toward Willimantic. Take the
second exit – marked for Eastern Connecticut State University/Route
195. At the end of the ramp, turn right onto High Street. Continue
on High Street to the end. The Town Hall is a large brick building
with a clock tower. It will be on your right at the intersection of High
Street and Main Street.
From FAIRFIELD COUNTY, New Haven and Bridgeport:
Take I-95 to New Haven. Take I-91 north to Hartford. Take Exit 29
(to Route 84 east – over the Connecticut River) and continue until
you merge with I-84 east. Take I-84 east through Hartford to Route
384 (Exit 59). Pick up
Route 6 in Bolton. At the intersection of Routes 6 and 66 in

From NORWICH Area:
Follow Route 32 to Willimantic. Cross over the “Frog Bridge”.
Continue straight after crossing the bridge and take a left at the
next light (Valley Street). Stay on Valley Street until you come to the
intersection of High Street. Turn left onto High Street. The Town Hall
will be on your right at the end of the block.
From Points NORTH:
Take Route 91 south to Enfield/Rte 190 exit. Follow Route 190 east
to Stafford Springs and the junction of Route 32 south. Follow Route
32 south to Willimantic City line (the cemetery will be on your right
and the highway overpass will be directly in front of you). At the
stoplight, take a left on Route 6 east. Go to next exit, which is
marked for Eastern Connecticut State University/Route 195. At the
end of the ramp, turn right onto High Street. Continue on High
Street to the end. The Town Hall is a large brick building with a
clock tower. It will be on your right at the intersection of High Street
and Main Street.
From Danielson and Points EAST:
Take Route 6 west into Willimantic. After Wal-Mart, take right fork
onto Rte. 6 toward Hartford. Get off at next exit. Turn left at light.
Turn left at next light onto High Street. Continue on High Street to
the end. The Town Hall is a large brick building with a clock tower.
It will be on your right at the intersection of High Street and Main
Street.
