SUMMARY
This workshop will teach you how to conduct a comprehensive
accident investigation. It discusses the reasons why accidents
should be investigated, the types of accidents that must be
investigated, and when an accident investigation should be
conducted. The workshop also discusses who should perform
the investigation.

LEARNING OBJECTIVES
Supervisors play a key role in accident investigations for
employees who are injured on the job. The purpose of an accident
investigation is to prevent the recurrence of the accident. The
focus should be on the accident, not the injury. Unfortunately,
too often, the investigation process is limited to the necessary
insurance paperwork.
Accident investigations are critical in identifying, controlling
and eliminating workplace health and safety risks. For an
accident investigation to be effective it needs to be structured
and systematic. It also needs to be performed by trained
employees who can maintain an open mind about the causes of
an accident throughout the investigation. These are fundamental
requirements of an accident investigation.
The primary learning objectives of this workshop are to:
- Understand the value of and need for accident investigation.
- Obtain the practical supervisory skills to conduct quality
accident investigations.
- Understand the critical role of the supervisor in the accident
investigation and reporting process.
- Learn how to determine the root cause(s) of accidents and
develop accident prevention action.
- Understand the CIRMA claims management function and
how investigations are critical to their success.

Luc Pentz
Risk Management Consultant, CIRMA
Luc Pentz serves as a Risk Management
Consultant for CIRMA’s Risk Management
Services. Luc has worked at CIRMA for the past
three years developing and implementing special
projects, products, and consultative services. Luc
holds a B.S. Degree in Criminal Justice from Quinnipiac University,
and is currently pursuing his Masters in Business Administration
and Associates in Risk Management designation through Quinnipiac
University’s School of Business. Luc provides proactive Risk
Management consultation for CIRMA’s Workers’ Compensation
and Liability-Auto-Property membership.

DIRECTIONS
From I-395 NORTH: Take Exit 92. Continue straight off the exit for about
a half-mile. Turn left onto Westcott Road (State Highway 607). Go about
one-half mile, then turn left onto Main Street/Route 12. Town Hall will be on
the left.
From I-395 SOUTH: Take Exit 92. Turn right at the end of the ramp onto
Westcott Road. Go about one-half mile, then turn left onto Main Street/
Route 12. Town Hall will be on the left.
From I-84 EAST: Take Exit 69 and a right onto Route 74. At the end of 74
take a left onto Route 44. Route 44 then runs into Route 101E (do NOT stay
on Rte 44, take Rte 101). At the intersection of Routes 12 and 101 take a
right onto Route 12. Continue on Route 12 for about 2.5 miles (will become
Main Street). Town Hall will be on the left.
From ROUTE 6 EAST: Follow Route 6 into Danielson, take a left onto
Route 12. Continue on Route 12/Main Street for about one-half mile. Town Hall
will be on the right.
From ROUTE 2 (Toward Norwich): Follow signs to I-395 North. Take Exit 92. Continue straight off the exit for about a half-mile. Turn left onto Westcott Road (State Highway 607). Go about one-half mile, then turn left onto Main Street/Route 12. Town Hall will be on the left.
